Thank you for trusting us with your care. To enhance patient services, North Georgia Eye Associates plans to close our Clayton office at 773 N. Main Street effective February 27, 2025. While this was a difficult decision, it allows us to further strengthen care across our practice. We remain fully committed to serving you and welcome you to visit any of our other five locations.

4. What happens to my scheduled appointment?

If you had an appointment scheduled at the Clayton office after the closure date, our staff will reach out to you to reschedule at one of our other locations. If you have not been contacted, please call us at (770) 532-4444 for assistance.

5. Will my doctor still be available at another location?

Many of our providers see patients at multiple locations. If you would like to continue care with your current doctor, please call us to confirm their availability at another office.

6. What will happen to patient medical records?

Your medical records will remain securely with North Georgia Eye Associates. If you would like to transfer your records to another provider, you can request a records transfer by completing a form, which is available at the front desk at any of our locations.

7. How do I request my medical records?

You can obtain a records request form from any of our offices. Our team is happy to assist with the process to ensure a smooth transition.
